What is a consent form?
A consent form is a written record that a person was told what they were agreeing to, understood it, and agreed to it freely. It is not a waiver and it does not transfer away anybody's rights — its job is to prove that a properly informed decision was made, on a specific date, by a named person.
That is why a signature on its own is worth very little. What makes a consent record hold up — in an ethics review, a complaint, or a dispute years later — is whether the document itself set out the purpose, the risks, the alternatives and the right to walk away, in language the signer could read in the time available.
