Does a patient have to sign to receive treatment?

No. A competent adult can consent verbally, and can refuse treatment entirely. The written form is evidence of consent, not a precondition for care.

Who signs for an adult who cannot make decisions?

No one signs on their behalf unless they hold a health and welfare Lasting Power of Attorney or court deputyship. Otherwise clinicians complete form 4 as a best-interests decision.
