Consent Forms is a free library of consent form templates, published by one person rather than a faceless company. It started after watching researchers, parents and small clinics waste hours hunting for a document that should take thirty seconds to find — then hitting an email wall or a subscription at the moment of download.

How the content is produced
Every template and explanation is written originally, then cross-checked against the official primary sources cited on each page — NHS England consent guidance, GOV.UK travel rules, HHS guidance on HIPAA authorisations, and equivalent bodies in Canada, Australia and New Zealand.
We study the structure and legal purpose of established forms and write our own wording from that understanding. We do not copy text from university PDFs, hospital trust paperwork or other template sites. The full process is documented in our editorial policy.
What we publish
Seven areas people search for most: interview and research consent, medical and patient consent, travel and minor consent, the UK NHS consent form family, photo and video releases, liability waivers, and HIPAA authorisations. Each template comes with a plain-English guide covering who signs, what the form must contain, and the mistakes that most often make a consent record useless.
How the downloads work
Documents are generated entirely in your browser. When you press "Download PDF" or "Download Word", the file is assembled on your device and saved directly. Nothing you type is transmitted to us, and we keep no copy of anything you produce — see the privacy policy.
How this site is funded
Everything is free with no signup, no email capture and no paywall. There are no affiliate placements inside the guidance. If advertising is ever added, it will be clearly separated from editorial content.
